I want to know how can I use Extension tools to change Session Name for both the text on the tab (title) and the text in the menu without duplicating from the standard component.

You can create a menu extension and change the description as it appears in the menu. As of 10.6 you can change the session title using form personalization.

I tried changing the Session Title using Form Personalization in 10.6. It works for the session part but in the menu part where it should show the description of the session, it shows the old description. It is actually refers to the session with its session code, not the menu. How can I change this session title in the menu too?

I was not clear in my post. For the menu you always need a menu extension, see screenshot. Before 10.6 it was not possible to change the session title on the tab.
