I am attempting to add forms fields that are not tied to table fields on a custom form, however, they look like labels and not text boxes.

At first I didn't see them at all until I wrote a little code that assigned a value to the variable the form field is bound to. Once I did that, I could see the data but I cannot change the data.

Never mind. I just discovered the Modify option was unchecked.
