We use PCS to configure Customized items (Projects). The only way we are able to do this (as far as we know) is to Generate the Variant Structure on the Sales Order line.

Anyone create PCS Projects without having a Sales Order? If so how is it done?

Manually create a PCS project and copy or insert an item in the PCS project. You do not need a sales order. Include final item as project part, calculate and change status to active.
